Thanks for booking a lactation consultation with me!
A few quick things before we meet.

My fees are due at the time of service (unless you’re an Aetna subscriber- then you have no out-of-pocket cost other than travel fee if applicable). You can pay via cash, check, PayPal, Visa, MasterCard, American Express, or Discover Card. At the end of the visit I provide you with a superbill that can be submitted to your insurance company for reimbursement for my services; unfortunately I can’t guarantee that they’ll do so. I do not offer refunds for services rendered for any reason.

The best way to find out if your insurance company will reimburse for my services is to call the member services number on the back of your insurance ID card and ask about your coverage for lactation or breastfeeding help. I am an out-of-network provider. Sometimes they will ask you for a procedure code or diagnosis code; generally I use 99404-33 for a procedure code and Z39.1 for a diagnosis code.

Please read my page about what to expect during a home visit. I promise you that I do NOT care if your house is messy. Pinky swear.
